Professional Disclosure and Intake Forms
In an effort to create a paperless practice, all my intake forms and disclosure notices will be sent to you via a confidential link.

Insurance
I do not accept insurance or offer statements for insurance coverage.

Fees
Individuals: $150 for a fifty-five minute session for individuals.
Couples: $200 for a fifty-five minute session.
Assessment fee structure discussed during initial session.

Payment

Confidentiality
What you share in the session is held in the strictest of confidence. I keep limited records and may take brief notes in session. Due to my exempt status, my records cannot be subpoenaed by the courts or other professional entities. I do not submit statements or records to insurance companies, which ensures a further level of confidentiality.

As a mandated reporter I am required to report if I suspect someone is at risk of harming themselves or others.

Cancellation Policy
I have a 24 hour cancellation policy. I reserve your session time especially for you. If you can’t give a 24 hour notice to cancel your appointment, you are responsible for the payment of that session.
